fa تأثیرات ورود الکترونیکی دستورات پزشک بر کاهش خطاهای تجویز دارو: یک مرور دامنه‌ای The Effects of Computerized Physician Order Entry on Reducing Medication Ordering Errors: A Scoping Review داروسازی داروسازی مروری Review ایمنی بیمار یک موضوع حیاتی در مراقبت‌های بهداشتی است که به جلوگیری از خطاهای دارویی و آسیب‌های ناخواسته می‌پردازد. فناوری‌های اطلاعاتی، به‌ویژه سیستم‌های ورود الکترونیکی دستور پزشک (CPOE)، می‌توانند به طور قابل توجهی خطاهای دارویی را کاهش دهند. هدف این مطالعه، انجام یک مرور دامنه‌ای جامع پیرامون تأثیرات ثبت کامپیوتری دستور پزشک بر کاهش خطاهای تجویز دارو می‌باشد. در این مطالعه مروری دامنه‌ای، مطالعات منتشر شده از سال 1999 تا 2024 در پایگاه‌های اطلاعاتی ScienceDirect، Google Scholar و PubMed با استفاده از کلمات و عبارات کلیدی "medication errors"، "CPOE"، "Computerized Physician Order Entry"، "Expert System"، و "reduction" جستجو شدند. از میان 13934 مطالعه بررسی‌شده در مرحله اولیه، در نهایت 11 مطالعه مرتبط واجد شرایط، وارد این مرور شدند. نتایج مطالعات مرور شده نشان داد که استفاده از سیستم‌های CPOE، به طور مؤثری، خطاهای تجویز را کاهش می‌دهد. CPOE به پزشکان این امکان را می‌دهد تا دستورات دارویی را به صورت دیجیتال وارد کنند و با کاهش ورود دستی اطلاعات، خطاهای تجویز را به حداقل برساند. ترکیب CPOE با سیستم‌های پشتیبان تصمیم‌گیری بالینی (CDSS) می‌تواند ایمنی بیمار را بهبود بخشد، اما ممکن است خود منجر به خطاهای جدید شود. بنابراین، نظارت مستمر و بهبود مداوم این سیستم‌ها برای جلوگیری از بروز خطاهای جدید مرتبط با سیستم، ضروری است. Patient safety remains a critical objective in healthcare, fundamentally addressing the mitigation of medication errors and associated unintentional patient harm. Computerized Physician Order Entry (CPOE) systems, a key health information technology, possess significant potential to substantially reduce these errors. This investigation was undertaken as a comprehensive Scoping Review to systematically map and analyze the effect of CPOE implementation on the reduction of medication ordering errors. A systematic search of the literature published between 1999 and 2024 was performed across the ScienceDirect, Google Scholar, and PubMed databases. The search strategy utilized key phrases, including "medication errors," "CPOE," "Computerized Physician Order Entry," "expert system," and "reduction." Following an initial screening of 13,934 publications, a final selection of 11 relevant and qualified studies satisfied the predetermined inclusion criteria for the review. The findings demonstrate that the implementation of CPOE systems is effective in minimizing prescription errors. CPOE facilitates the digital input of medication orders by prescribing physicians, thereby reducing the human transcription errors associated with manual data entry. Furthermore, integrating CPOE with Clinical Decision Support Systems (CDSS) offers enhanced patient safety; nevertheless, this integration may inadvertently introduce new, system-related vulnerabilities.
