<p align="justify" style="DIRECTION: rtl">ویروس هپاتیت B یکی از مهم ترین مشکلات بهداشتی انسان در جهان می باشد و باعث هپاتیت حاد و مزمن<span dir="rtl"></span><span dir="rtl"></span><span lang="FA" style="LINE-HEIGHT: 115% FONT-FAMILY: " dir="rtl"><span dir="rtl"></span><span dir="rtl"></span>،</span> سیروز کبدی و یا هپاتوسلولار کارسینوما می شود. ویروس هپاتیت B شامل یک DNA دو رشته ای حلقوی و پوشش دار است که به 8 ژنوتیپ مختلف طبقه بندی می شود(A-H). شیوع ژنوتیپ های مختلف HBV در مناطق مختلف متفاوت بوده و ژنوتیپ غالب در ایران ژنوتیپ D می باشد. براساس گزارش سازمان بهداشت جهانی بیش از دو میلیارد نفر در دنیا با این ویروس مواجهه دارند و در حال حاضر بیش از 350 میلیون نفر یعنی قریب 5 درصد از انسان های کره زمین این ویروس را با خود حمل می کنند و عده ای از آنها مبتلا به هپاتیت مزمن و یا نارسایی و سیروز و یا سرطان کبدی بوده. سالیانه عامل مرگ یک میلیون نفر و منبع آلودگی مهمی برای دیگران می باشد. شیوع هپاتیت B در دنیا 20-1/0 درصد می باشد که که به سه گروه تقسیم می شوند. 1- مناطقی دارای شیوع کمتر از 2 درصد، 2- مناطقی دارای شیوع 7-2 درصد، 3-مناطقی با بیش از 8 درصد. طبق بررسی های انجام شده در ایران 8/3-3/1 درصد افراد ناقل مزمن این ویروس می باشند. با شناسایی این ویروس و راه های انتقال آن دانشمندان در صدد تهیه واکسن برای پیشگیری و کشف داروهایی جهت درمان آن برآمدند که از حدود سه دهه قبل عملی و موفقیت های زیادی نیز کسب شده است. واکسیناسیون علیه این ویروس از سال 1372 در کشور ما شروع شد و در حال حاضر یکی از 9 واکسن مرسوم و الزامی برای کودکان ایران می باشد. در رابطه با پیشگیری از عفونت هپاتیت B از طریق ایمن سازی فعال همه اتفاق نظر دارند ولی در مورد چگونگی شروع درمان و تنوع دارویی و طول مدت درمان اختلاف نظر وجود دارد. هدف مقاله حاضر مروری بر هپاتیت B مزمن، نظرات موجود در رابطه با زمان شروع درمان، داروهای موجود و نحوه استفاده از آن و چگونگی پیگیری بیماران می باشد.</p>
Hepatitis B virus (HBV) infection is a major global health problem and is a factor for
progressive liver diseases such as chronic hepatitis, cirrhosis and epatocellular carcinoma.
HBV is an envelope, double stranded DNA virus with a genome of approximately 3200 base
pairs.
It has been classified into eight major genotypes (A-H) and multiple subtypes, and HBV
genotypes have distinct geographical distribution of the world. WHO estimation of HBV
prevalence, suggests that there are nearly two billion people worldwide who have been
infected with this virus and more than 350 million of people live with chronic infection (up
to 5% of the world population), of these near one million people die annually from
complications of chronic hepatitis B, which is the 10th leading cause of death worldwide.
Throughout the world, carrier variability rate for hepatitis B infection is estimated to be 0.1%
to 20%, with regions classified as having low (<2%), intermediate (2-7%) and high (>8%)
endemic. In Iran, it is estimated that near 30% of population have been exposed to HBV and
the endemicity is intermediate, with a carrier rate 1.3%-3.8% in the different provinces of
this country. In Iran, the HBV vaccination program has been started in 1993 and after that,
Iran can be considered as one of the countries with low HBV infection endemicity.
The most genotype circulating in some provinces of Iran were genotype D in different
clinical forms of HBV infections. The distribution of HBV genotypes may guide us in
determining disease burden.
It is important to have information about the epidemiology of HBV as response to therapy
and vaccination to control HBV infection. Therefore, its successful treatment has major value
in quality of life of patients infected by HBV. This study designed a systematic review of
chronic HBV infection to determine the successful treatment and following HBV positive
patients.
