Panic disorder sufferers are at risk of severe decline in quality of life and exacerbation of symptoms. This study aimed to predict panic attacks based on brain-behavioral systems and anxiety in panic attack sufferers with the mediation of the abandonment schema variable. This research, classified as descriptive and correlational based on structural equation modeling, focused on outpatients with panic attack experiences who visited emergency departments in Karaj City hospitals in 2022. Using a purposeful sampling method, individuals with a history of panic attacks were selected according to inclusion criteria, voluntary participation, and consent. Following Plant's formula (8M + 50 ≤ N), the sample size was determined at 140. Inclusion criteria were the absence of anxiety disorder and major depression, no use of psychoactive drugs, and voluntary cooperation with consent, verified through a structured interview. Data collection involved a demographic information checklist and researcher-developed forms from the Panic Disorder Symptoms Questionnaire (Libutiz et al., 1984), Brain-Behavioral System Questionnaire (Carver and White, 1994), Beck Anxiety Questionnaire, and Young's Schema Questionnaire. Data analysis was conducted using Pearson correlation methods and structural equation modeling with SPSSV19 and EMOS20 software. The results indicated that behavioral brain systems and anxiety variables had a direct and significant effect on the severity of panic attacks. The anxiety variable also indirectly and significantly affected the severity of panic attacks through the mediation of the abandonment schema. However, brain-behavioral systems did not have an indirect effect on panic attacks through the mediation of the abandonment schema. The structural model of panic attacks based on behavioral brain systems and anxiety, with the mediation of the abandonment schema, demonstrated a good fit index. Therefore, this model can assist doctors and counselors in improving the provision of services to individuals presenting with panic attacks.
