This study aimed to present a model for health-oriented leadership with an organizational behavior knowledge approach in government organizations of Razavi Khorasan Province, Iran. This study was conducted using a sequential exploratory mixed-methods type, with the qualitative section utilizing the grounded theory method and interviews with 14 experts until data saturation. The quantitative section used a survey method with 383 managers and employees completing the questionnaire via stratified sampling. The research instrument was a researcher-made questionnaire with 82 items across 6 dimensions and 12 components, the validity and reliability of which were confirmed. SPSS software (version 27) and PLS software (version 3.2) were used for data analysis and structural equation modeling. The findings indicated that the final model has good fitness. "Health-oriented leadership" was placed as the core phenomenon at the center of the model, shaped by causal conditions (training and structural barriers), contextual conditions (organizational culture and financial resources), and intervening conditions (leadership support and resistance to change). The extracted strategies in transformational and structural dimensions played a key mediating role in achieving individual and organizational outcomes. Structural equation results showed that strategies had the strongest impact (coefficient 0.716) on the core phenomenon and, in turn, influenced strengthening organizational culture and employee training with coefficients of 0.858 and 0.824, respectively. Additionally, health-oriented leadership had a direct effect (coefficient 0.550) on organizational outcomes, which in turn led to improved organizational performance and increased job satisfaction with coefficients of 0.884 and 0.895, respectively. The final model evaluation with the Q² index confirmed moderate to good fitness for all dimensions. This model can serve as a scientific framework for policy-making and planning to establish health-oriented leadership in government organizations.
